Care Navigator Service - West Kent

Sevenoaks District Care Navigator Service

In partnership with Kent County Council (KCC) we have secured funding for a Care Navigator Service for the Sevenoaks district.

This is a free community service which supports older people who are undergoing change in their lives and may need information and help accessing services. The aims of the service are to reduce social isolation, improve economic well being, manage physical and mental health, maintain an independent lifestyle in their own home and live safely within their communities. We want them to feel in control and be at the centre of any decision about their welfare.

Examples of the advice we can offer include:

  • benefits advice
  • work and training opportunities
  • housing advice 
  • physical and mental health issues
  • aids and adaptations
  • minor works
  • community activities
  • leisure activities
  • citizenship.

We can also help direct people being discharged from Sevenoaks, Tunbridge Wells, Darent Valley and Princess Royal University hospitals to other organisations, agencies and relevant services. We take referrals from individuals and agencies working with older people.
